Warfarin Care Program

Requirements for registration are that the patient or carer must have a mobile phone to receive results and that the patient is able to understand and follow dosing instructions.

Nursing homes have a choice of receiving results via download, fax or web portal.

4Cyte Pathology is not responsible for providing Warfarin care should a patient be admitted to hospital. Patients will need to have a Hospital Discharge form completed (below), to recommence in the warfarin program post hospitalisation.

Patients who are repeatedly non-compliant with Warfarin care instructions will be discharged from our Warfarin care program.

NB - Warfarin Care is currently only offered to patients residing in QLD, VIC and Northern Rivers.
