To prevent unauthorised access to the portal, 4Cyte will verify the identity of the requesting NUM or lead consultant using their personal identifiers:

Declaration

I, on behalf of the Hospital/Hospital ward or department listed above:

• Request access for the Hospital/Hospital ward or department to 4Cyte Pathology's Web Results Portal;
• Request access to all pathology results held by 4Cyte Pathology in the Web Results Portal, regardless of date or referring practitioner;
• Acknowledge that, in obtaining access to the Web Results Portal, staff accessing the portal will have access to significant volumes of sensitive personal information (including health information);
• Will use all reasonable endeavours to ensure that the Web Results Portal is only used to access pathology results necessary for the proper treatment of patients of the Hospital/Hospital Ward or department who have consented to that access occurring and for no other purpose;
• Will otherwise use all reasonable endeavours to ensure that there is no:
• Misuse, interference or loss; or
• Unauthorised access, modification or disclosure of or to pathology results held in 4Cytes Pathology's Web Results Portal
• Undertake to ensure that 4Cyte Pathology is immediately notified if there is an “eligible data breach” for the purposes of the Privacy Act 1988 in connection with pathology results held in the Web Results Portal as a consequence of the Hospital's access to that portal; and
• Acknowledge that use of access to the Web Results Portal will be monitored and that access may be deactivated if 4Cyte Pathology reasonably suspects misuse of the portal
